VCSE Programme Director

The ambition of the Humber, Coast and Vale (HCV) Partnership is for everyone in our area to: start well, live well and age well

This means shifting the focus of our work from picking people up when they fall to helping to prevent them from becoming unwell in the first place and supporting more people to manage their health and wellbeing at home so they can get on with living happy and fulfilling lives.

To do this, we need to look at how we deliver health and social care services working in partnership with other sectors including the VCSE sector, a key partner in delivering transformation of health and wellbeing in our region

An exciting new position of Programme Director has been created to ensure the VCSE sector has a strong influential voice and visibility, and is able to contribute to the health and well-being of the Humber Coast and Vale population.

You will be part of a dynamic team in delivering the effective implementation of the Humber Coast and Vale third sector input across the partnership as well as managing the day to day activity of the VCS Leadership Programme.

A natural collaborator, the successful candidate will quickly establish credible and professional relationships with colleagues across the partnership to deliver the transformation required.

You will have significant experience at a senior level in leading and managing change, an understanding of, and experience of working with the VCSE sector, ideally in the context of improving health and wellbeing outcomes and an understanding and knowledge of the workings of the NHS, the challenges it faces and of the reform agenda.

Primary Care Link Worker

We have an opening for a Primary Care Link Worker to join our existing team here at York Medical Group (YMG). The ideal candidate should be friendly and approachable, have excellent customer services, good IT skills and be a flexible team player.

Main duties include:
Under the direction of York CVS implement patient referrals from within YMG and provide a personalised package of support to individuals as appropriate. Develop good working relationships with colleagues and GP’s in order to identify patients who would benefit from the promotion of social prescribing. Build and maintain effective partnership with voluntary sector providers and statutory services and highlight gaps in services for development. Working across a significant level of multi-agencies, support patients in a planned and structured way by co-producing personalised care plans to improve health and wellbeing outcomes. Ability to engage with patients in one to one appointments but also to facilitate group work. Keep accurate patient records and product outcomes and data as required for data collection and reporting.
