Ways to Wellbeing (social prescribing) Practitioner

  • Fixed term to 31st March 2022 
  • Salary: £25,417 per annum pro rata 
  • Hours: 37 per week (but for the right candidate we may consider fewer hours) 
  • Secondment will be considered

Due to the rapid success of this expanding project, we are recruiting a Social Prescribing Practitioner for the Ways to Wellbeing social prescribing project based at York CVS.  

Ways to Wellbeing has been providing social prescribing across the City of York for a number of years, and is one of York’s two social prescribing projects. Read more on the York CVS website here . 

Ways to Wellbeing Practitioners work closely with NHS and local authority services, as well as community and voluntary sector groups. This role will primarily be to support individuals after their discharge from NHS secondary care, to help reduce the number of re-admissions to services. 

The successful candidate will have significant experience of working with vulnerable adults in health and social care settings eg. NHS, local authority services or VCSE sector. They must have a passion for the principles of social prescribing, excellent communication and organisational skills, the experience and ability to manage their own caseload and a creative enthusiasm for joining us in growing social prescribing in York.

VCSE Programme Manager

  • (7 month secondment opportunity or fixed term contract)
  • Circa £35k
  • 37 hours per week

The ambition of the Humber, Coast and Vale (HCV) Partnership is for everyone in our area to: start well, live well and age well. This means shifting the focus of our work from picking people up when they fall to helping to prevent them from becoming unwell in the first place and supporting more people to manage their health and wellbeing at home so they can get on with living happy and fulfilling lives.

To do this, we need to look at how we deliver health and social care services working in partnership with other sectors including the VCSE sector, a key partner in delivering transformation of health and wellbeing in our region. An exciting new position of Programme Manager (full-time) has been created to support the Programme Director in implementing the emerging VCSE Strategic Plan particularly at a place level, increasing its connectivity to local organisations and networks.

A natural collaborator, the successful candidate will quickly establish credible and professional relationships with colleagues across the partnership to deliver the transformation required.

You will have significant experience of managing and building relationships and networks. You will have an understanding of, and experience of working with the VCSE sector, ideally in the context of improving health and wellbeing outcomes and an understanding and knowledge of the workings of the NHS, the challenges it faces and of the reform agenda.
